Any one wishing to refer a student to Malden Oaks for the following types of placement should complete Achieving for Children’s Alternative Placement Panel (APPP) Referral Form:

• Dual Registration placement
• Day 6 Provision (Permanent Exclusions)
• Secondary Alternative (Medical) Provision
• Secondary Alternative (SEN) Porvision
• Any queries about this should be addressed to Achieving for Children directly.
