Dear Parent/Carer

The Kingston School Health Service provides a range of services to children and young people in Kingston. My name is Nicky and I am a specialist practice school nurse working with Malden Oaks.

I offer students a personal health assessment to identify any unmet health and wellbeing needs and to look at ongoing support they may benefit from. This will be completed during school hours. I also provide a confidential health drop in once a week, which students are free to access.

The assessment is confidential but if students are happy for information to be shared with you to support their needs I will make contact after the assessment. If safety concerns are identified during the assessment then the student will be informed about the need for information to be shared to ensure
that they are kept safe.

The assessment and records about your child will be recorded on the secure Your Healthcare record keeping system. This information will form part of your child’s complete medical record.
